Remove all mention of the OMC DC PD chain (which no longer uses this whitening board). This effectively undoes revisions 6 and 7.
Fix the BOM error in the differential input and replace R15 and R21 that were stuffed with 3kΩ thick film resistors with metal film ones.
Remove the 1uH inductors at the input of the power supply. Together with the bypass capacitors these inductors form a resonant circuit near 10kHz which results in a noise peak at this frequency.
Added a separate solid state switch at the input of the gain and filter stages. This prevents these stages from being over-driven and generating excess noise, when they are bypassed. See https://alog.ligo-wa.caltech.edu/aLOG/index.php?callRep=84435 for more details.
