Starting in CY 2013 all Operations Change Requests (CRs) are to be "R" documents. Note that Advanced LIGO change requests (ACRs) are still "M" documents.

This version of the Operations Change Request form has been changed as follows:
1) Changed the document number field from "E" to "R"
2) Added a CR title field
